Declaro que he sido informado(a) adecuadamente y entiendo las características de la actividad física y ejercicio individualizado que se ofrece en los Centros de Acondicionamiento y Preparación Física de Simetrica Pravilo, dirigido por Anastasiia Yukhymchuk, NIE Y6871242Y. Entiendo que mi participación implica riesgos físicos, incluidos, pero no limitados a, lesiones personales, y por lo tanto declaro que mi estado de salud es adecuado para tales actividades.

I declare that I have been properly informed and understand the characteristics of the physical activity and individualized exercise offered at Simetrica Pravilo Fitness and Conditioning Centers, led by Anastasiia Yukhymchuk, NIE Y6871242Y. I understand that my participation involves physical risks, including, but not limited to, personal injuries, and therefore I declare that my health condition is suitable for such activities.

Además, confirmo que no padezco de ninguna de las siguientes condiciones, que son contraindicaciones principales para el entrenamiento en la "Regla" (Pravilo): Furthermore, I confirm that I do not suffer from any of the following conditions, which are the main contraindications for training on the "Pravilo":

• Tumores y quistes en el cuerpo, especialmente de naturaleza maligna. / Tumors and cysts in the body, especially of a malignant nature.
• Etapas agudas de enfermedades articulares y de la columna. / Acute stages of joint and spine diseases.
• Fracturas por compresión y lesiones recientes en la columna. / Compression fractures and recent spinal injuries.
• Cirugías recientes. / Recently undergone surgeries.
• Presencia de clavijas, implantes y otras construcciones en el sistema musculoesquelético. / Presence of pins, implants, and other constructions in the musculoskeletal system.

• Procesos inflamatorios e infecciosos agudos en el cuerpo. / Any acute inflammatory and infectious processes in the body.

• Patologías cardiovasculares severas. / Severe cardiovascular pathologies.
• Enfermedades mentales severas, incluyendo la epilepsia. / Severe mental illnesses, including epilepsy.
• Embarazo y período menstrual en mujeres. / Pregnancy and menstrual period in women.
• Riesgo de sangrado. / Risk of bleeding.
• Presión arterial alta, dolor de cabeza agudo o mareos. / High blood pressure, acute headache, or dizziness.
• Peso extremo. / Extreme weight.

Me comprometo a informar inmediatamente al personal de Simetrica Pravilo sobre cualquier dolor, incomodidad, fatiga, u otro síntoma que pueda afectar mi salud o ponerla en riesgo antes, durante, y después de mi participación en cualquiera de las actividades y servicios ofrecidos.

I commit to immediately inform the staff of Simetrica Pravilo about any pain, discomfort, fatigue, or any other symptom that may affect my health or put it at risk before, during, and after my participation in any of the activities and services offered.

Entiendo y acepto que cualquier omisión de información sobre mi estado de salud exonera y libera de toda responsabilidad a Simetrica Pravilo, Anastasiia Yukhymchuk, y su personal, por cualquier accidente, lesión o situación adversa que pudiera surgir de mi participación en las actividades ofrecidas.

I understand and accept that any omission of information about my health condition exempts and releases Simetrica Pravilo, Anastasiia Yukhymchuk, and its staff from any liability for any accident, injury, or adverse situation that may arise from my participation in the offered activities.
